Each Circle Home hospice patient is assigned to a primary nurse, who coordinates the hospice team's services to that patient. Our nurses have years of experience and expertise in helping patients manage symptoms such as pain, nausea, anxiety and other discomforts. Most of our nurses are nationally certified in hospice and palliative care.
Our hospice nurses assess and monitor the patient's condition, coach patients and caregivers about medications, suggest ways for patients to easily manage daily living, support and inform family caregivers, assist in problem-solving, and communicate with the Hospice Medical Director. Their goal is to help patients remain as alert and active as possible, feeling the best that they can every day.
A hospice nurse is always available to answer questions or to make a home visit if needed.
