Merrimack Valley Hospice is committed to continuing improvement of services. We use a variety of strategies to assess and improve the quality of our hospice services. Based on collected data, we identify opportunities for improvement and develop and implement action plans. Notes from our biweekly team meetings, medical record nursing reports, and patient/family/colleague calls and comments are consistently reviewed.
The perceptions of family caregivers about the care their loved one received is especially important to us. After the death of the patient, the family caregivers receive an anonymous survey and results are compiled to identify future improvement efforts. Some recent comments:
