What is home health care?

Home health care is a professional health service that is delivered to patients in their homes. Our home health care "on the road" staff includes nurses, rehabilitation therapists, social workers and home health aides.

Home health care makes it possible for many people to recover from or adapt to a health challenge, while they are in the convenient and comfortable surroundings of their home. Home health care is designed to assist patients of all ages who have an injury or illness and are expected to fully recover; those who are learning how to live well with a chronic disease, such as heart disease or diabetes; and those with advanced illness who are receiving treatments and palliative care.

Home health care is typically initiated after a hospital or rehab facility stay, when continued assessment, treatment, or therapy is needed at home. Patients are also referred when they have a major health change or a new and serious diagnosis. Education for the patient and the family caregivers is an important part of home health care services.

Home health care requires a referral and ongoing direction from a physician.
